As a Social Prescribing Link Worker, you will work closely with primary care teams to enable individuals to find the support they need in their community to manage their health and well-being. You will also enable people to have more control over their lives, develop skills and give their time to others through involvement in community groups. You will see people in their local GP practice and community venues and may visit people in their homes.

This includes strength-based, trauma-aware and person-centred approaches. You will have experience of managing your own caseload, including supporting people who have complex issues affecting their health and wellbeing The ability to adhere to relevant legislation, good practice, and policies and procedures, including Child Protection, Safeguarding Adults, Health and Safety, Confidentiality, and Equality and Diversity is also essential.
